you can find a brief discussion of the theorm in samuel laio's 'microwave devices and circuits' chapter 9 associated with the analysis of the TWT and periodic structures in general ,also you can find a good insight in collin's 'foundations for microwave engineering ' 2nd edition in chapter 8 it's all about periodic structures
