There are some filters that are used for some commercial applications such as DECT, GSM, UMTS etc...
So these filters are easily available and cheap but if you intend to use a special charaterirized filer e.g. particular bandwidth, insertion loss, bandstop attenuation, group delay etc. you have to pay some money to make them manufacture.
This is called as "tooling"..It's very expensive...
Yep, what he said. Saw filters are readily available at 70 Mhz, 140 MHz, PCS and cell phone frequency bands. Saw resonators are available at sonnet frequencies, etc.

Primary SAW vendors in the world are Epcos, Murata and Tai-SAW (in Taiwan). From my personal experience, Tai-SAW can also do custom design, and the price is <S30K. You can also find other, relatively smaller vendors (for example, check out RFM). Epcos and Murata are the biggest SAW players in the world today. They all have in-house foundries.
SAW devices are differential in nature. Interface is typically differential, and can be described as a resistor parallel to a capacitor. Most vendors issue reference designs, where they "overcome" the differential architecture by means of external BALUNs. HOWEVER, I believe you can just approach the vendor directly, as ask for a differential interface model of the SAW component.
