Designing a non-reciprocal and lossy S matrix device

I need a device that has the following S matrix. How can I do that?
..... [0 0 0]
S = [1 0 0]
..... [1 0 0]

Your design has given me an idea. I would use it to combine 2 linear ortogonally polarized double ridged waveguides into a quad ridged one. However, I can change it so that the amplifier works as its input and output is the same quadruple ridged waveguide. Can I do it? Is it possible?
