Can someone recommend a good reference book on balun theory and design using passive components? Thanks.

Frankly, I have not yet seen any book focusing on balun design. But you can search online for some documents regarding to balun design.

There is a book "Understanding, Building, and Using Baluns and Ununs" , attached a little pdf help.
