DEIC515 MOSFET Driver

Anyone ever use this DEIC515 to drive the RF MOSFET? I'm really confused with its 8pins and wonder how should I connect them...One more problem is what is CMOS and TTL input? Its needed for the input of DEIC515 based on its datasheet...can I supply the input just by using function generator?
