Recommended Reading for Patch Antennas, and advice on material dimensions

I have an intuitive understanding of what is going on with a patch antenna, but I would like to read about the calculation of different design parameters so I can do some pen and paper calculations/designs before going to the lab to use the CST software.

1) Could you recommend papers available online, or websites where I can get a good discussion on microstrip patch antenna design please? including the calculations and, as much as possible, where the calculations came from.

2) I understand this is a question with a million answers, but what might be a 'real-world' measurement for the thickness of the dielectric and the two copper layers?
