I have two questions:
1. MMIC layout design
There are some books on MMIC design theory. I understand that layout can be designed and simulated using some software. There are also limitations of manufacturing process.
What book or author you can suggest, what software is currently trending?
2. MMIC manufacturing
Where MMIC can be manufactured, what approximate prices?
What are prototype manufacturing costs?
Also there is a good topic clarifying difference between Microwave integrated Circuits(MIC) and Monolithic MIC(MMIC):

Can MIC manufacturing be cheaper than MMIC?

MMIC Layout Design is pretty similar to RFIC Layout Design.Design Rules,Layout Topologies etc. are of course different but the scheme behind them is same. For MMIC layout design softwares are simply Keysight ADS ( my preference ) and AWR.
There are foundries such as QORVO ( former Triquint and RFMD ), WIN ( Taiwan based foundry, they got many processes for different applications), Analog Devices ( Hittite), GCS ( They have very good processes for Low Noise Oscillators ) and others.
It's hard to say something about costs without knowing your specific application.
