To solve Text book's example by HFSS

Hello everybody,

Today I try to solve the example of electromagnetics textbook, but I meet a serious problem because I am not good at HFSS.
I think I need to know what kind of boundary condition and port should be adopted.
I attach the HFSS file and problems what I tried.
