Antenna with electric tilt feature
Are you referring to an antenna with a fixed electrical downtilt?
If so, that antenna will have multiple internal elements with a phase difference between them. To the user though, there is still just one RF connection and the user does not change the phase, the phase offset between elements is done inside the antenna.
Your antenna must be an array of elements (at least two). Most commonly you have some kind of phase control for each element for beam steering.
It is done for mortar radars, so the radar can track an incoming device even though it is falling rapidly,- too fast to track with a scanner.

electrical tilt is basically applied on an antenna when the changes to feed signal is done!
